Paradigm Healthcare in Wharton, TX is seeking a motivated LVN/RN to join our team as MDS Coordinator. This full-time Day shift role focuses on maintaining MDS status and coordinating assessments with the interdisciplinary team.

You will lead ARD meetings, ensure regulatory compliance, and support residents and families while delivering high-quality care in a collaborative environment. EOE.

Competenze

• Must possess a current, unencumbered, active license to practice as an LVN or RN in TX.
• RN license preferred.
• 2 years’ Long-Term Care experience preferred.
• 6 months in rehabilitative/restorative nursing preferred.
• 2 years’ experience in RAI process preferred.
• Ability to make independent decisions with leadership and organization.
• Strong PC skills and familiarity with Point Click Care.

Mansioni

• Maintain the current MDS status of each resident per laws and facility policy.
• Schedule residents for OBRA, Medicare PPS, and/or Medicaid assessments and inform departments and families.
• Monitor MDS process changes and coordinate reassessments.
• Set ARD dates with the interdisciplinary team to reflect needs and services.
• Complete MDS and CAAs accurately per guidelines.
• Attend daily Casemix, weekly UR, and monthly Triple Check meetings.
• Maintain assessments for 15 months per Federal Regulations.
• Transmit MDS data to the state electronically per policy.
• Educate residents, families, and caregivers as needed.
• Provide CPR and emergency support as required.
