Job summary

Qualifications

• Must hold a current LVN/LPN license in the practicing state.
• Prior LTC or skilled nursing facility experience preferred.
• Knowledge of MDS 3.0, RAI process, and Medicare/Medicaid guidelines required.

Responsibilities

• Complete MDS assessments and CAAs within required timelines.
• Ensure accuracy of resident documentation to support care planning and reimbursement.
• Maintain CMS/state/facility compliance and document regulatory guidelines.
• Collaborate with interdisciplinary team to develop individualized care plans.
