Job summary

Allure of Zion is seeking an experienced MDS Coordinator to join our skilled nursing facility. The position focuses on accurate and timely Medicare/Medicaid MDS documentation to ensure proper reimbursement for care and services provided.

As a RN/LPN MDS Coordinator, you will coordinate interdisciplinary participation, maintain MDS schedules, and handle data entry and electronic submissions. We require current RN licensure and LTC experience, plus strong English communication skills.

Qualifications

• Registered Nurse with current unencumbered state licensure.
• Long Term Care Experience preferred
• Ability to read, write, speak, and understand the English language
• Ability to sit, stand, bend, and walk regularly; lift and/or move up to 25 pounds

Responsibilities

• Assess and determine the health status and level of care of all new admissions
• Ensure the accurate and timely completion of all MDS Assessments, including PPS Medicare, quarterly, annual, and significant change
• Coordinate interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission to facility
• Maintain an accurate schedule of all MDS assessments to include proper reference dates throughout the resident's stay
• Responsible for data entry and electronic submission of MDS assessments
• Verify electronic submissions of MDS, perform corrections when necessary, and maintain appropriate records
