What Will You Do in This Role?

As an INTERIM MDS Coordinator | LPN, you will be responsible for the timely and accurate completion of the RAI process and care management from admission to discharge, following company policies and federal, state, and certification guidelines. You will handle data tracking such as Minimum Data Set, discharge, and admission records. Under the guidance of the Director of Nursing and VP of Clinical Reimbursement, you may also coordinate information systems operations and staff education within the clinical department.

Do You Have What It Takes?

• A passion for working with seniors
• A flexible, energetic personality
• Current LPN license in practicing state
• Prior MDS experience preferred
• At least one year of experience in LTC/SNF/AL/MC settings preferred
• Knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement preferred
• Understanding of Quality Indicator processes, OBRA regulations, and MDS
• Care planning knowledge required
• Professional appearance and behavior
• Excellent communication skills
