Job summary

Avardis Health is looking for an experienced MDS Coordinator (LPN) to oversee resident assessments and ensure accurate documentation for optimal care planning and reimbursement at Starkville ManorHealth Care and Rehabilitation.

The role requires an active LPN license, 6+ months of MDS coordination experience, and a focus on compliant, patient-centered care. Preference given to candidates with RAC-CT or RNAC certification and SNF/Rehabilitation experience.

Qualifications

• Active LPN license in good standing.
• 6+ months of experience as an MDS Coordinator.
• Experience in Skilled Nursing/Rehabilitation preferred.
• RAC-CT or RNAC certification preferred.

Responsibilities

• Oversee the coordination and participate in the completion of the Resident Assessment Instrument (MDS, CAA's, and Care Plan) in compliance with Federal and State regulations.
• Monitor and document the management of Medicare and Managed Care residents, working closely with the interdisciplinary team.
• Ensure accuracy in resident assessments to support reimbursement and high-quality care.
• Drive innovation by implementing new ideas and process improvements.
• Stay up to date with regulations and best practices to maintain compliance.
