Job summary

Fundamental is seeking an experienced LPN in Rock Hill, SC to coordinate the MDS process and support the DON in delivering high-quality resident care.

The MDS Nurse will ensure accurate data collection for the Minimum Data Set and contribute to the Interdisciplinary Plan of Care while collaborating with the nursing team and other disciplines. Experience with MDS/RAP and PPS processes is preferred.

Qualifications

• Must have a current, unencumbered LPN license (as required by state).
• 1+ years of direct hands-on nursing and MDS/RAP experience preferred.
• Ability to utilize both local and consultant resources in the execution of job responsibilities.
• Must have the ability to develop an adequate knowledge of state, federal, and regulatory requirements related to the job.
• Reliable transportation required

Responsibilities

• Coordinate the Resident Assessment Instrument (RAI) process.
• Complete an accurate Minimum Data Set (MDS) and develop the interdisciplinary Plan of Care.
• Participate in the daily Medicare Prospective Payment System (PPS) Interdisciplinary Team Meeting (ITM).
• Establish and maintain positive relationships with all members of the healthcare team as part of the nursing management team.
