Job summary

Liberty Healthcare & Rehabilitation Services seeks an experienced MDS Coordinator (LPN license required) to lead MDS processes and resident care planning. The role requires coordinating across disciplines, ensuring timely state submissions, and maintaining accurate documentation.

You will complete MDS, CAAs, and RCPs, and conduct regular team meetings to discuss resident needs and care plans, under the supervision of the DON/Administrator.

Qualifications

• Licensed Practical Nurse with NC license and current renewal proof.
• Ability to read and follow policies, procedures, and state/federal requirements.
• Experience with MDS-RAP and care planning functions.
• Preference for experience with RUG-IV.
• Preference for experience with MDS 3.0.

Responsibilities

• Maintain and follow a schedule of due dates for all MDS.
• Coordinate MDS completion across disciplines.
• Review MDS data for accuracy and meet with staff as needed.
• Complete MDS and input into the computer.
• Verify timely submission/approval by State; correct and resubmit as needed.
• Complete CAAs per state guidelines.
• Formulate Resident Care Plans (RCP) with the interdisciplinary team.
• Schedule and conduct regular RCP meetings.
• Communicate RCP to staff and notify supervisor of noncompliance.
• Collaborate with departments to ensure RCP understanding and compliance.
• Perform related duties as directed by DON/Administrator.

MDS COORDINATOR (LPN LICENSE REQUIRED)

Job Description

• Maintains and follows a schedule of due dates for all MDS.
• Coordinates the completion of the MDS by all disciplines.
• Reviews the MDS data for accuracy and meets with appropriate staff as needed to assure accuracy.
• Completes the MDS and inputs into the computer.
• Verifies that assessments have been transmitted and approved by the State in a timely manner.
• Corrects any rejected records and prepares them for re-submission.
• Completes CAAs according to state guidelines.
• Formulates Resident Care Plan (RCP) along with interdisciplinary care plan team for all residents in accordance with their needs and within the required time frame.
• Schedules and conducts RCP meetings on a regular and timely basis.
• Communicates the RCP to appropriate staff members and notifies supervisor if noncompliance by staff is noted.
• Works with all in house and ancillary departments to assure understanding and compliance with the RCP.
• Performs other related duties as directed by the DON and -or Administrator.

Job Requirements

• Licensed Practical Nurse, graduated from an accredited School of Nursing and have a current, valid LPN license from the North Carolina Board of Nursing and submit proof of license renewal every other year.
• Ability to read, know, and follow personnel, department and facility policies and procedures and adhere to local state and federal requirements.
• Experience with MDS-RAP and Care Planning functions.
• Prefer experience with RUG-IV.
• Prefer experience with MDS 3.0.
