Unity Health and Rehabilitation Center in Florida is seeking an experienced MDS Coordinator to join our care team. You will lead data analysis to develop comprehensive care plans and coordinate with physicians, therapists, and nursing staff to ensure high-quality resident care.

The ideal candidate holds a Florida RN license (or equivalent) and has at least 1 year in a long-term care setting. You will maintain accurate records, monitor progress, and contribute to quality improvement initiatives.

About the Role:

As an MDS Coordinator, you will be responsible for:

• Monitoring patient progress and making adjustments to care plans as necessary

• Familiarity with MDS software and electronic medical records
