Job summary

Nazareth Living Care Center in El Paso, TX seeks an LVN or RN to join us as MDS Coordinator, full time, day shift. The role focuses on MDS status, ARD planning, and coordinating assessments with the inter-disciplinary team.

You will complete MDS/CAAs, transmit data to the state, participate in Casemix and Utilization meetings, and educate residents and families as needed. Must have current LVN/RN license in Texas; RN license preferred; LTC/RAI experience is a plus.

Qualifications

• Must possess a current, unencumbered, active license to practice as an LVN or RN in the State.
• A Registered Nurse License is preferred.
• 2 years’ Long-Term Care experience preferred.
• 6 months of rehabilitative and restorative nursing experience preferred.
• 2 years’ experience in RAI process completion preferred.

Responsibilities

• Maintain the current MDS status of each resident according to laws, rules, and facility policies.
• Schedule residents for OBRA, Medicare PPS, and/or Medicaid required assessments and communicate the schedule.
• Monitor changes in resident condition affecting MDS; coordinate reassessment and plan of care updates.
• Set ARD with interdisciplinary team to reflect needs and services provided.
• Complete MDS and CAAs per policies and guidelines; transmit data to state as required.
• Participate in daily Casemix Meetings, weekly Utilization Review Meetings, and monthly Triple Check Meetings.
• Maintain assessments for a minimum of 15 months as required by Federal Regulations.
• Provide education to residents, families, and caregivers when needed.
