Cantex Continuing Care Network is seeking a MDS Coordinator in Flower Mound, TX. You will oversee the RAI process, ensure compliant MDS assessments, and coordinate care plans to support Medicare/Medicaid reimbursement.

The role emphasizes regulatory compliance, interdisciplinary collaboration, and accurate documentation to maintain high-quality resident care. A Texas nursing license is required and certification within a year is expected.

Qualifications

• Current Texas nursing license (RN or LVN) is required.
• Must complete AANAC RAI Certification within 1 year of employment.
• Must have HHSC RUG Online Training for Nursing Facilities (Acknowledgement of Completion).
• Strong communication, report writing, and ability to interpret regulations and care plans.

Responsibilities

• Maintain compliance with State and Federal Medicaid/Medicare rules.
• Coordinate MDS assessments and care plan coordination.
• Audit clinical records for accurate documentation of care delivery.
• Lead pre-admission eligibility assessment discussions.
• Coordinate reimbursement meetings with interdisciplinary team to ensure accurate billing.
