Job summary

The Arboretum of Winnie is seeking an MDS Coordinator to ensure accurate assessments and compliant care planning for our residents. In this full-time role you will coordinate admissions, annual and change-of-condition MDSs, lead interdisciplinary meetings, and monitor documentation for accuracy and timeliness.

Qualifications

• Must hold a current nursing license and be in good standing.
• Experience in geriatric nursing and resident-centered care.
• Excellent assessment, documentation, and communication skills.
• Understanding of nursing home regulations and compliance.
• Ability to work independently and in a team; manage priorities.

Responsibilities

• Coordinate Resident Assessments: admissions, annual, quarterly, Medicare, change-of-condition with timely interdepartmental communication.
• Ensure Regulatory Compliance: follow Federal and State guidelines for MDS.
• Monitor Documentation: review MDS and care plan files for accuracy and organization.
• Utilize MDS Software: maintain data entry and reporting accuracy.
• Lead Interdisciplinary Meetings: coordinate Medicaid, ITTM, and care plan meetings.
• Update Care Plans: revise plans based on assessments and resident status.
• Provide Quality Reporting: generate and distribute Quality Indicators Reports to leadership.
• Submit MDS Data: ensure timely and accurate transmission per standards.
• Stay Current: participate in professional development to keep up with industry standards.
• Oversee Ancillary Data: report ancillary services to billing for accurate processing.
