Job summary

Crest Pointe Rehabilitation and Healthcare Center is seeking an experienced MDS Coordinator to oversee the timely and accurate completion of Minimum Data Set assessments. The role supports resident care planning, reimbursement, and regulatory compliance while collaborating with interdisciplinary teams and leadership.

Ideal candidates hold an active RN license with a nursing degree and have at least three years of clinical experience in a long-term care setting, including MDS/RAI familiarity.

Qualifications

• MDS Coordinator role requires accurate MDS timing, regulatory compliance, and collaboration with care teams.
• Strong clinical assessment skills with attention to detail and accuracy in long-term care settings.
• Experience with Medicare/Medicaid reimbursements and RAI/MDS guidelines is essential.

Responsibilities

• Ensure timely and accurate completion of all MDS assessments per deadlines.
• Verify compliance with federal, state, and facility regulations.
• Supervise MDS data entry, validation, and transmission for accuracy and timeliness.
• Identify and resolve data discrepancies and validation issues.
• Prepare MDS reports for DON and leadership.
• Support audits and regulatory reviews with required documentation.
