Job summary

Cape Heritage Rehabilitation & Health Care Center in Sandwich, MA, is seeking an MDS Coordinator to plan, organize, and direct the MDS process for high-quality resident care.

The role requires a nursing degree and active MA LPN/RN license, with English proficiency and the ability to make independent decisions. Competitive pay ranges from $42 to $50 per hour, depending on experience and licensure, with benefits and career advancement opportunities.

Qualifications

• Must possess a nursing degree from an accredited school of nursing, college, or university.
• Must possess an active and unencumbered license to practice as an LPN or RN in Massachusetts.
• Must be able to read, write, speak, and understand the English language.
• Must possess the ability to make independent decisions and deal tactfully with various stakeholders.
• Must be knowledgeable of nursing and medical practices, as well as laws and regulations that pertain to long-term care.

Responsibilities

• Coordinate the MDS process, including overseeing resident care plans and completing MDS reports.
• Oversee ADL training for facility staff
• Issue and deliver denial notices and Medicare denial letters in a timely and appropriate manner
• Complete and transmit all CMS-approved item sets (MDS) accurately and efficiently
• Maintain knowledge of and comply with current CMS regulatory guidelines
• Ensure the confidentiality of resident health information and maintain adequate systems to support billed services
