Are you an experienced MDS nurse interested in the next step? The MDS Coordinator provides oversight of the RAI process, conducts assessments, and coordinates care plans for guests. The role supervises the Care Management Nurse and MDS Nurse.

Responsibilities:

  • Completes MDS, CAA’s, and care plans within regulated time frames.
  • Coordinates scheduling of the RAI process with the interdisciplinary team.
  • Assesses residents through physical assessments, interviews, and chart reviews.
  • Discusses resident care needs with caregivers, including physicians, nursing, social services, therapy, dietary, and activity staff.
  • Reviews information from hospitals, consults, and outside agencies, using this information in assessments and care planning.
  • Coordinates, identifies, and initiates significant changes in MDS assessments.
  • Prepares for PPS meetings, maintains MDS assessments per Medicare schedule, and monitors Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
  • Remains current with AANAC requirements.

Qualifications:

  • Registered Nurse (RN)
  • AANC certification is a plus; RAC-CT preferred
  • Knowledge of the Resident Assessment Instrument (RAI) process and the Prospective Payment Process (PPS)
  • Experience as an MDS Nurse
