Job summary

Ceres PostAcute Care in California is seeking an experienced MDS Coordinator to direct, organize and schedule resident assessments for Medicare, Medicaid and private insurance reimbursement with the interdisciplinary team.

You will calculate Medicare assessments and PPS dates, oversee documentation to comply with California reimbursement rules, review medical records, attend care plan conferences, and ensure timely, accurate MDS submissions while staying current with professional developments.

Qualifications

• Licensed California RN or LVN required
• Minimum 1 year experience as an MDS Nurse
• English communication skills required for business communication
• Administrative and organizational abilities essential
• CPR certification preferred
• Knowledge of PointClickCare

Responsibilities

• Collaborates with all departments to facilitate completion of MDS process
• Calculates Medicare assessments and PPS dates
• Oversees documentation to comply with Medicare or California Dept. of Health reimbursement rules
• Review medical records and related information for MDS records
• Calculate quarterly annual assessments with appropriate dates
• Monitors input to ensure accuracy, timeliness and compliance with reimbursement authorities and standards
• Attends care plan conferences as needed and Medicare meetings
• Assess resident care needs and assist in development of individualized care plans
• Keeps current with job related professional developments, regulations and standards
• Attend and participate in continuing education programs to keep abreast of changes in the profession
• Identifies problems and reports for corrective action
• Cooperates with residents, staff, volunteers, and families to ensure quality care
• Observe all company policies and procedures
