Job summary

Hibbard Skilled Nursing and Rehabilitation Center in Dover-Foxcroft, Maine seeks a skilled MDS Coordinator to lead resident assessments and ensure compliance with state and federal regulations. The role reports to the Director of Nursing and supports high-quality long-term care.

The ideal candidate will have MDS and ICD-10 coding experience, Maine RN or LPN licensure, and strong assessment and care-planning abilities to uphold our standards of care.

Qualifications

• MDS and ICD-10 coding experience preferred; RAC-CT certification preferred.
• Current, active Maine RN licensure or current active Maine LPN licensure.
• Strong nursing assessment, care planning and delivery of care skills for long-term care.

Responsibilities

• Assess/Oversee the RAI process and ensure coding accuracy.
• Review care plans for accuracy and case-mix validity.
• Oversee and complete care plans per nursing standards, including restorative plans.
• Establish documentation approaches and instruct staff to document properly.
• Communicate MDS due dates to the interdisciplinary team and ensure timely completion.
• Instruct disciplines on MDS items, CAAs and Quality Indicators.
