Job summary

iCare Health Network is seeking an experienced MDS Coordinator in Connecticut to lead the MDS process and ensure accurate reimbursement through PPS/OBRA compliance. The role requires coordinating interdisciplinary teams and maintaining up-to-date MDS documentation.

The ideal candidate holds an active CT nursing license (RN preferred) and brings supervisory or lead experience in long-term care, along with familiarity with MDS/PPS updates and staff training.

Qualifications

• Active nursing license in CT (RN preferred).
• 2+ years supervisory or lead experience in long-term care.
• 3 months MDS experience or training in PPS/MDS.

Responsibilities

• Coordinate assessment, planning, implementation and evaluation of resident care by following the MDS process.
• Coordinate and facilitate care conferences with the interdisciplinary team.
• Complete, review and ensure accuracy of Minimum Data Sets (MDS).
• Educate and train nursing staff on MDS roles, PPS, and related sections.
• Ensure MDS/PPS updates are implemented and staff are in-serviced.
• Cross-train between PPS and OBRA.
• Respond to clinical emergencies, regulatory visits, and surveys outside normal hours.
