Job summary

Fresh River Healthcare is seeking an MDS Coordinator to oversee the Medicare/Medicaid MDS process, ensuring accurate documentation and maximizing reimbursement. This role requires coordinating assessments across nursing, dietary, social services and rehabilitation teams.

Responsibilities include leading care conferences, completing MDS, and training staff on MDS requirements while responding to emergencies and regulatory visits as needed.

Qualifications

  • Active RN or LPN license in Connecticut.
  • Knowledge of QA/Improvements processes.
  • Bachelor's/Associate degree in a health care related field preferred.
  • Experience in supervision in long-term care preferred.
  • 3 months of MDS experience or completed MDS training.

Responsibilities

  • Coordinate assessment, planning, implementation and evaluation of resident care following the MDS process.
  • Facilitate care conferences with the interdisciplinary team.
  • Complete and review Minimum Data Sets (MDS).
  • Educate and train nursing staff and other departments on MDS processes.
  • Ensure MDS/PPS updates are implemented and staff are trained.
  • Cross-train between PPS and OBRA.
  • Conduct regular resident interviews as required by MDS.
  • Respond to emergencies and regulatory visits outside normal hours.

Job description

MDS Coordinator

The primary role of the MDS Coordinator is to coordinate and guide the overall process and tracking of all Medicare/Medicaid case-mix documents, and care assessments in order to assure appropriate reimbursement for services. The position is also responsible for completing concurrent MDS to assure achievement of maximum allowable RUG categories, as well as integrating information from Nursing, Dietary, Social Services, and Rehabilitation departments for appropriate reimbursement.

Responsibilities:
  • Coordinates the assessment, planning, implementation and evaluation of resident care by following the MDS process.
  • Coordinates and facilitates care conferences with the interdisciplinary team.
  • Coordinates, completes and reviews Minimum Data Sets (MDS)
  • Educates, trains and assists nursing staff and other departments regarding the roles of assessments, planning, implementation and MDS process; and completion of applicable sections of MDS.
  • Responsible for ensuring all current MDS/PPS updates are implemented and staff is in-serviced.
  • Will cross train between PPS and OBRA.
  • Conducts regular resident interviews as required by the MDS
  • Available to respond to clinical emergencies, regulatory agency visits, surveys and issues outside of normal work hours as needed
Qualifications:
  • Active RN or LPN license in the State of Connecticut, required.
  • Knowledge of Quality Assurance/Improvement processes, required.
  • Bachelors or Associate degree in a health care related field, preferred.
  • Experience in a supervisory position in long-term care, preferred.
  • Requires at least 3 months experience completing MDS and completion of MDS training program or equivalent experience and knowledge of PPS
