Overview:

We are seeking a dedicated and detail-oriented MDS Coordinator (RN) to join our skilled nursing facility. The ideal candidate will be responsible for assessing the nursing care needs of residents, coordinating the MDS/MMDS program, and ensuring compliance with all State and Federal regulations. This role plays a critical part in developing personalized care plans and supporting optimal reimbursement through accurate documentation and submission.

  • Analyze nursing care needs of residents and develop individualized care plans to guide nursing staff in providing personalized, quality care.
  • Implement, coordinate, and monitor the MDS/MMDS program to maintain full compliance with State and Federal regulations.
  • Set up and maintain a monthly Care Plan schedule to ensure timely evaluations and updates.
  • Participate actively in nursing committees and departmental meetings as needed.
  • Stay current with industry best practices by attending seminars, workshops, and continuing education opportunities.
  • Collaborate professionally and tactfully with residents, families, facility staff, Administration, Director of Nursing (DON), and consultants.
  • Ensure initial and ongoing care plans are consistently updated, reflecting accurate resident status.
  • Accurately transmit MDS information to the State within mandated timeframes.
  • Facilitate medical assistance reviews and ensure complete and appropriate documentation to support the level of care and reimbursement processes.
  • Conduct ADL coding audits related to MDS completion and compliance; submit findings to the Administrator and DON for review and appropriate action.
  • Assist the DON in reporting quarterly audit results to the QAPI Committee.
  • Participate as a nursing team member in implementing care plans developed through CAA protocols.
  • Review and verify accuracy of the CMS 672 and 802 forms monthly; communicate any discrepancies to the DON and provide final reports to the Administrator and Executive Director.
