We are seeking a dedicated and experienced MDS Coordinator to join our dynamic healthcare team at Woburn Rehabilitation and Nursing Center.

Responsibilities
- Conduct and coordinate the development and completion of comprehensive resident assessments
- Conduct resident assessments in accordance with current state and federal rules, regulations, and guidelines
- Implement resident Care Area Assessments (CAAs)
- Coordinate and support the development and implementation of individualized resident care plans
- Collaborate with the interdisciplinary care team to develop comprehensive assessments and care plans
- Participate in facility survey readiness and throughout the survey process
- Ensure MDS-related documentation is accurate, complete, and submitted within required timelines
- Communicate effectively with nursing staff, physicians, therapists, social workers, and other interdisciplinary team members
- Support compliance with applicable regulatory requirements and facility policies
Qualifications
- Current, unencumbered Massachusetts RN or LPN license
- Minimum of two years of MDS and PPS experience
- Previous care planning experience required
- Strong assessment, documentation, and organizational skills
- Excellent communication and interpersonal skills
- Ability to work effectively with an interdisciplinary team
- Strong knowledge of applicable MDS, PPS, and regulatory requirements
