Job summary

Berkshire Nursing & Rehabilitation center in West Babylon, NY is seeking an MDS Coordinator to join our interdisciplinary team of skilled health care professionals at our skilled nursing facility. This is an excellent opportunity for an RN looking to move into a nonclinical role.

Responsibilities include completing accurate MDS and care plans, monitoring documentation, initiating and updating care plans, and auditing clinical records while protecting confidentiality.

Qualifications

• Experience with MDS 3.0 preferred.
• Valid NY State RN license.
• Long Term Care experience preferred.
• Highly organized, professional & eager to learn.
• Solid computer skills.
• Excellent communication skills.
• Detail oriented and motivated; training provided.
