Autumn Lake Healthcare at Salem County is seeking an MDS Coordinator to join a supportive team in New Jersey. The role involves coordinating resident assessments and ensuring accurate MDS processes to support high-quality care.

Qualified candidates will hold a NJ LPN or RN license with at least one year of experience in a hospital, long-term care facility, or related setting, with a preference for prior MDS coordination work.

Qualifications

• NJ or multi-state LPN or RN license required.
• Minimum 1 year of experience in a hospital, long-term care facility, or other related health care facility.
• Previous experience as an MDS Coordinator preferred.
