Autumn Lake Healthcare at Spa Creek is seeking a dedicated MDS Coordinator to join our compassionate team. The role focuses on accurate MDS assessments, care planning, and regulatory compliance to support residents and staff.

Applicants should hold a Nursing degree and have at least 1 year of experience in a hospital, long-term care facility, or related health care setting. Previous MDS Coordinator experience is preferred. We offer training and a supportive management team to help you grow.
