Job summary

Beauvais Rehab and Healthcare Center seeks an experienced LPN/RN MDS Coordinator to join our leadership team. This role ensures compliance and maintains our high standard of resident care, supported by upper management and a commitment to excellence.

Responsibilities include coordinating RAIs, collaborating on individualized care plans, and providing QA data. Missouri license required; prior MDS coordination experience preferred.

Qualifications

• Active and current LPN or RN license in Missouri.
• Previous experience as an MDS Coordinator.
• Strong understanding of Medicare/Medicaid regulations and benefit guidelines.

Responsibilities

• Conduct and coordinate RAIs per federal, state, and local regulations.
• Collaborate with Interdisciplinary Care Plan Team to develop resident-specific care plans.
• Evaluate resident conditions and ensure all special assessments are completed as required.
• Provide data for the Facility’s Quality Assurance Program.
