Job summary

American Medical Associates seeks an experienced MDS Coordinator to join our nursing home team in Sodus, NY. You will coordinate resident assessments (MDS) and work with care plans, ensuring timely completion and accuracy.

The role requires an active New York RN license, MDS 3.0 knowledge, and prior long-term care experience. You will collaborate with directors and the assessment team to optimize resident care and compliance.

Qualifications

• Must hold active New York RN license.
• Must have MDS Coordinator experience.
• Must have long-term care experience.
• Must know MDS 3.0 and Care Plan process; PPS and RUGs knowledge.
• Must participate in quarterly and annual assessments.

Responsibilities

• Conduct and coordinate resident assessments (MDS).
• Maintain and update written policies and procedures that implement MDS and care plan.
• Assist the resident in completing the care plan portion of the resident’s discharge plan.
• Develop and implement procedures with the Director of Nursing Services to inform all assessment team members of the arrival of newly admitted residents.
• Assist Facility directors and supervisors in scheduling the resident assessment and care plan meetings.
• Assist in determining appropriate treatment, selecting activities and exercises based on medical and social history of residents.
• Participate in the development and implementation of resident assessments (MDS) and care plans, including quarterly and annual reviews.

Salary: $95K to $100K range; based on experience
