We are hiring an MDS Coordinator to join our dynamic care team at Cape Heritage Rehabilitation & Health Care Center in Sandwich!
Shift is 8a-4p with weekly pay. Can be an LPN or RN.

As the MDS Coordinator, you will help plan, organize, and direct the MDS process, overseeing resident care plans through clinical assessment, review of medical history, personal interviews, and completion of MDS reports.
Experience & Education:
- Must possess a Nursing Degree from an accredited school; RN preferred.
- Must hold a current, unencumbered RN or LPN license in this state.
Duties & Responsibilities:
- Coordinate the MDS process
- Oversee ADL training for staff
- Issue and deliver denial notices timely
- Complete and transmit CMS-approved MDS item sets
- Follow CMS regulatory guidelines as per RAI Manual
- Protect residents’ health information
- Maintain Medicare meeting minutes
- Manage Medicare/Insurance benefits
- Ensure documentation supports billed services
- Perform other duties as assigned
Specific Requirements:
- Proficient in English
- Ability to make independent decisions
- Tactful communication with personnel, residents, families, agencies
- Ability to coordinate assessment and care plans in a timely manner
- Knowledge of nursing practices, laws, regulations
- Leadership and supervisory skills
- Patience, tact, cheerful disposition, enthusiasm
- Willingness to learn and incorporate new methods
- Effective communication during emergencies
- Accurate assessment skills
