Job summary

Sequoyah Pointe Living Center in Owasso, OK is seeking an RN with long-term care experience to join our team as the MDS Coordinator. The role focuses on completing care plans and assessments to finalize the MDS.

You will schedule meetings with residents and families, communicate with staff, and advocate for residents. Comprehensive training will be provided; MDS coordinators are not pulled to the floor.

Qualifications

• Active RN license is required.
• Knowledge of PDPM is required.
• PCC experience preferred.
• Professional appearance and attitude required.

Responsibilities

• Complete all care plans and assessments needed to finalize the MDS.
• Interview and communicate effectively with residents.
• Set up calendar schedule for care plan meetings with residents, families, and MDS due dates.
• Act as a resident advocate and coordinate with staff; training provided to ensure success.

Job Type: Full-time

In this position, you will be responsible for completing all care plans and assessments needed to complete the MDS. As the MDS coordinator you must be able to effectively communicate with and interview our residents. You will be required to set up an effective calendar schedule for care plan meetings with residents & family members & MDS due dates. You will be a resident advocate! As an MDS coordinator we expect professionalism & for you to be able to communicate with other staff members. Comprehensive training will be provided to assure your success as an MDS Coordinator with our company.

Our MDS coordinators are NOT pulled to the floor as a nurse!
