Moose Lake Village, a Cassia community, is seeking an experienced RN to join as a Full-Time MDS Coordinator. You will oversee MDS/care planning and coordinate nursing tasks on the unit, ensuring accurate documentation and timely assessments.

The role requires MN RN licensure, RAC certification, MDS experience, and strong computer skills. Competitive pay and site-specific benefits accompany a supportive team environment.

Responsibilities

• Oversee and complete MDS and care planning.

• Develop initial and temporary care plans.
• Assist Case Manager with Medicare scheduling, monthly billing, charting, and issuing notices of non-coverage.
• Document and maintain paperwork, referral records, and reports.
• Work in collaboration with Admissions staff, Case Manager, and Supply Coordinator.
• Display a courteous attitude and respect for all residents, families, and staff.
• Perform additional tasks as needed.

Job description

Moose Lake Village, a Cassia community, has a rare opportunity for an experienced Registered Nurse (RN) to join our team as a Full-Time MDS Coordinator.

In this role, you will manage the clinical care of residents by assigning different tasks to nurses within the department. Our ideal candidate has prior LTC experience, thrives in a fast-paced environment, excels in multitasking, and has excellent customer service skills.

Position Type: Full-Time, benefits eligible position
Day Shifts: 8:30am to 5:00pm
Wage Range: $40 - $48 / hour depending on experience
Location: 710 S Kenwood Ave Moose Lake, MN 55767

MDS Coordinator Responsibilities:
