Cassia Community's Moose Lake Village is seeking an experienced RN to serve as a Full-Time MDS Coordinator, overseeing MDS and care planning for residents in a 72-bed facility.

The role requires MN RN licensure and RAC certification, with LTC experience and solid computer skills to support case management and coordination across departments.

Qualifications

• RN license in Minnesota required.
• Resident Assessment Coordinator certification required.
• MDS experience preferred.
• Proficient in computer software.

Responsibilities

• Oversee and complete MDS and care planning.
• Manage and assign tasks to nursing staff on the unit.
• Complete all documentation for RAI and MDS assessments and observations.
• Develop initial and temporary care plans.
• Assist Case Manager with Medicare scheduling, monthly billing, charting, and notices of non-coverage.
• Document and maintain paperwork, referral records, and reports.
• Collaborate with Admissions staff, Case Manager, and Supply Coordinator.
• Display a courteous attitude and respect for all residents, families, and staff.

Job description

Moose Lake Village, a Cassia community,has a rare opportunity for an experienced Registered Nurse (RN) to join our team as a Full-Time MDS Coordinator.

In this role, you will manage the clinical care of residents by assigning different tasks to nurses within the department. Our ideal candidate haspriorLTC experience, thrives in a fast-paced environment, excels in multitasking, and has excellent customer service skills.

Position Type: Full-Time, benefits eligible position
Day Shifts: 8:30am to 5:00pm
Wage Range: $40 - $48 / hour depending on experience
Location: 710 S Kenwood Ave Moose Lake, MN 55767

MDS Coordinator Responsibilities:

• Oversee and complete MDS and care planning.
• Manage and assign tasks to nursing staff on the assigned unit.
• Complete all documentation for RAI and MDS assessments and observations.
