Allaire Health Services in Morristown, NJ is seeking an RN MDS Coordinator to lead the MDS processes, ensure accurate assessments, and coordinate care planning with the nursing team.

You will train staff on care plans and EMR usage, monitor EMR records for consistency, and communicate issues to leadership, while contributing to a supportive, patient-centered workplace.

• Must have working knowledge of nursing services, nursing administration, rehabilitation, general nursing, and geriatric nursing.
• Must be able to initiate, complete and update care plans with strong organizational and interpersonal skills.
• Experience conducting in-service training of nursing staff on care plans and EMR usage.
• Ability to monitor resident EMR records for consistency and content.
• Excellent communication with residents, families, and staff in a professional manner.

Responsabilidades

• Develop and update care plans based on MDS assessments.
• Coordinate MDS processes and timelines with the care team.
• Train nursing staff on care plans and EMR procedures.
• Monitor and review EMR records for accuracy and completeness.
• Communicate identified issues to the DON and relevant staff.
• Maintain a personable, cooperative relationship with residents and families.

MDS Coordinator Responsibilities

• Must have working knowledge of nursing services, nursing administration, rehabilitation, general nursing, geriatric nursing, the MDS, and its computer application (EMR)
• Must have good managerial, organizational, plannings, computer, and interpersonal ability to initiate, complete and update care plans.
• Ability to conduct in-service training of nursing staff on care plans, documentation, and EMR application.
• Ability to monitor resident EMR records for consistency and content.
• Ability to identify problems and to communicate those problems to the DON and staff concerned
• Be personable with residents, families, and staff in a professional and cooperative manner
