Job summary

Northern Metropolitan Center in Monsey, NY is seeking a MDS Coordinator and MDS Assessor to support nursing home residents and ensure compliance with MDS 3.0 processes.

The role involves completing assessments, developing and updating care plans, and maintaining accurate documentation while collaborating with the Director of Nursing and care teams. Strong organizational and communication skills are required, along with proficiency in patient records management.

Qualifications

• Must have prior MDS 3.0 experience.
• Valid New York RN license.
• Long Term Care experience required.
• Highly organized, professional, and motivated.

Responsibilities
