Job summary

Saint Therese is seeking an MDS/RAI Coordinator (RN) in Michigan to lead the RAI/MDS assessment process for quality resident care and regulatory compliance. You will work with nursing and interdisciplinary teams to ensure timely assessments, accurate documentation, and compliant care plans while supporting reimbursement processes.

The role requires strong knowledge of MDS/RAI, PPS, and Medicare regulations, plus proficiency with EHRs and analytics.

Qualifications

  • Current RN license in Michigan in good standing
  • 3+ years experience in long-term care
  • Experience with MDS/RAI and Case Mix
  • Experience with PPS, Medicare, HMOs and third-party payors
  • Care planning in long-term care
  • Strong documentation and regulatory knowledge
  • Proficient with Microsoft Office, EHR, ROX, and Excel

Responsibilities

  • Coordinate and complete the full Resident Assessment Instrument (RAI)/MDS process for assigned residents
  • Ensure assessments are completed timely and accurately in compliance with State and Federal regulations
  • Conduct resident interviews, participate in care conferences, and attend discharge meetings as needed
  • Apply knowledge of Case Mix, PPS, Medicare, HMOs, managed care, and third-party payors to support accurate reimbursement
  • Utilize ICD-9/ICD-10 coding to ensure accurate documentation and care planning
  • Review charts and clinical information to identify concerns and elevate appropriately
  • Partner with nursing and interdisciplinary teams to address quality of care opportunities
  • Support development of individualized care plans based on resident assessments
  • Contribute to regulatory readiness and quality outcomes
  • Maintain accurate records and reporting using Microsoft Office, EHR systems, ROX, and Excel
  • Prioritize workload to meet regulatory and organizational timelines
  • Support consistent workflows and documentation standards across the clinical team

  • -Maintain accurate records and reporting using Microsoft Office, EHR systems, ROX, and Excel
  • -Effectively prioritize workload to meet regulatory and organizational timelines
  • -Support consistent workflows and documentation standards across the clinical team
Minimum Qualifications
  • -Current licensure as a Registered Nurse (RN) in good standing with the Michigan Board of Nursing
  • -Minimum of three (3) years of experience in long-term care
  • -Demonstrated experience with MDS/RAI processes and Case Mix
  • -Experience working with PPS, Medicare, HMOs, managed care, and third-party payors
  • -Experience with care planning in a long-term care setting
  • -Strong understanding of clinical assessment processes, documentation standards, and regulatory requirements
  • -Proficiency with Microsoft Office, EHR systems, ROX, and Excel
  • -Strong written and verbal communication skills
  • -Ability to work both independently and collaboratively within interdisciplinary teams
  • -Excellent organizational, time management, and prioritization skills
  • -High level of professionalism, attention to detail, and commitment to quality care
Preferred Qualifications
  • -Certification in Gerontology
  • -Previous experience supporting quality improvement or regulatory readiness initiatives
