Position: MDS Coordinator
Location: Miami, FL
Salary: Up to $44 per hour

Are you an experienced RN with a passion for ensuring quality care in a skilled nursing facility? Azure Shores Rehabilitation, located in beautiful Miami, FL, is seeking a dedicated MDS Coordinator to join our team and help us deliver exceptional care to our long-term residents.

Key Responsibilities:

1. Ensure Quality Care: Oversee care standards to ensure our long-term residents receive the highest level of support.
2. Conduct Assessments: Perform detailed assessments and interviews for new residents to develop customized care plans.
3. Lead Care Planning: Design individualized care plans with measurable goals for both short-term and long-term resident improvement.
4. Collaborate Across Teams: Facilitate interdisciplinary meetings with healthcare professionals, families, and caregivers to create well-rounded care approaches.
5. Community Engagement: Work closely with specialists to drive the best possible outcomes for our residents.
6. Provide Leadership & Education: Lead nursing management meetings and deliver in-service training on MDS, RAPS, and Triggers, ensuring staff stay informed and proactive.
7. Maintain Compliance: Keep the MDS/Care Planning schedule on track, focusing on non-Medicare residents.

What We’re Looking For:

1. Licensed Registered Nurse (RN) in Florida
2. At least 2 years of experience in a skilled nursing facility (SNF)
3. Strong understanding of MDS, Medicare, and the care planning process
