Administrative Functions
- Administer patient assessments, oversee the assessment process, set assessment schedules, and ensure assessments are completed accurately and timely.
- Coordinate the care plan according to regulatory requirements.
- Create schedules for Medicare and Medicaid, initiate Medicare coverage for new residents, and handle denials and updates on Medicare coverage. Assist with Medicaid reimbursement documentation.
Supervisory and Departmental Responsibilities
- Direct the daily functions of nursing assistants in compliance with regulations and policies.
- Ensure all staff adhere to policies, periodically review and revise policies, and participate in shift planning and quality assurance programs.
- Participate in facility surveys and update resident discharge plans as needed.
Resident Care and Documentation
- Ensure rooms are ready for new admissions, greet residents, and assist with their orientation.
- Coordinate diagnostic and therapeutic services, review treatment plans, and ensure proper documentation and charting.
- Administer medications as ordered, verify resident identity, and maintain accurate narcotic records.
Personnel and Staff Development
- Participate in staff evaluations, staffing decisions, and training programs.
- Lead by example, conduct rounds, and ensure staff follow safety and sanitation protocols.
Care and Clinical Functions
- Implement and monitor care plans, assist with resident assessments, and carry out nursing procedures.
- Coordinate with physicians, monitor resident conditions, and ensure compliance with care standards.
Qualifications
- Licensed as an LPN/LVN with a current, active license.
- Minimum education: Nursing degree from an accredited program.
- Excellent communication, reasoning, and leadership skills.
Physical and Work Environment
- Ability to lift 25 pounds, move intermittently, and work in various environments within the facility.
- Must meet health requirements and be able to work beyond normal hours if needed.
