Job summary

Trevida Healthcare is seeking a dedicated MDS Coordinator to ensure timely and complete MDS, CAA's, and Interdisciplinary Care Plan processes. You will monitor assessments and guide inputting, coordinate state transmissions, and support care conference scheduling.

Requirements include a Nursing Degree and an active RN license; CPR is preferred and RAC-CT is a plus. Prior LT care experience as an MDS Coordinator is expected to join our team and contribute to high-quality resident care.

Qualifications

• Must hold a Nursing Degree from an accredited college or university.
• A current, unencumbered, active RN license and CPR preferred but will also consider an LPN.
• At least 1 year experience in Long Term Care as a MDS Coordinator.
• RAC-CT Preferred.

Responsibilities

• Monitors and guides the completion of assessments in a timely manner.
• Involvement and supervision of inputting of MDS assessments, the transmission to the state, and obtains receipt of validation.
• Schedules and facilitates care plan conferences; Facilitates monthly QM meetings.

MANDATORY QUALIFICATIONS OF MDS COORDINATOR:

SKILLS AND QUALITIES OF MDS COORDINATOR:

• Outstanding knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement.

• Excellent written and verbal communication skills and critical thinking abilities,

• Solid understanding of current federal, state, and local standards, regulations, and guidelines that govern our facility

• Ability to organize and prioritize
