Job summary

HERMITAGE NURSING & REHAB in Hermitage, Missouri is seeking an MDS Coordinator to join our team. The ideal candidate will complete accurate MDS assessments and care plans, initiate care planning activities, and maintain up-to-date documentation.

Qualified applicants hold an RN or LPN license and have prior MDS and long-term care experience, with strong organizational and communication skills. This full-time position offers benefits and a supportive team environment.

Qualifications

• MDS Experience in skilled nursing settings preferred.
• Experience with MDS and care planning processes is required.
• Long Term Care experience is required.
• Must be able to work 40 hours per week.
• Must be highly organized, professional & motivated.
• Solid computer skills are expected.
• Excellent communication skills are essential.
• Friendly and a team player attitude is needed.

Responsibilities

• Complete accurate MDS assessments and care plans as assigned.
• Initiate care plans and supporting activities as assigned.
• Create and distribute monthly care plan calendars timely.
• Maintain and update all care plans and assessments.
• Monitor and audit clinical records for accuracy and timeliness.
• Inform DON of persistent issues with documentation.
• Protect confidentiality of resident and facility information.
• Participate in meetings and committees (Medicare, QAPI, Weight Management).

HERMITAGE NURSING & REHAB, located in Hermitage, Missouri is currently seeking an MDS Coordinator to join our team.

MDS Coordinator Duties and Responsibilities:

• Completing accurate assessments, MDS & care plans as assigned

• Initiating care plans and supporting activities as assigned

• Creating and distributing monthly care plan calendars in a timely fashion

• Maintaining & updating all care plans and assessments as required

• Monitoring & auditing clinical records, ensuring accuracy & timeliness

• Informing DON of persistent issues related to non-compliant documentation

• Protecting the confidentiality of Resident & Facility information at all times

• Participates in various meetings and committees: Medicare, QAPI, Weight Management etc.

MDS Coordinator Qualifications and Skills:

• Valid RN or LPN License

• Knowledge of skilled nursing facilities survey process and regulations would be helpful.

• Must have prior MDS Experience

• Long Term Care Experience Required

• Must be able to work 40 hours a week

• Must be highly organized, professional & motivated

• Should have solid computer skills
