Job summary

Chelsea Place Care Center, located in Hartford, CT, seeks an MDS Coordinator to oversee Medicare/Medicaid case-mix documentation, MDS planning and care assessments to ensure optimal reimbursement. This role coordinates across Nursing, Dietary, Social Services and Rehabilitation teams, and ensures PPS updates are implemented.

The position requires an active CT RN or LPN license, knowledge of QA/Improvements, and MDS experience or training. Supervisory experience in long-term care is preferred.

Qualifications

• Active RN or LPN license in CT.
• Knowledge of QA/Improvement processes.
• Bachelor's or Associate degree in health care (preferred).
• Experience in supervisory position in long-term care (preferred).
• At least 3 months experience completing MDS or MDS training (or equivalent).

Responsibilities

• Coordinate assessment, planning, implementation and evaluation of resident care by following the MDS process.
• Coordinate and facilitate care conferences with the interdisciplinary team.
• Complete and review Minimum Data Sets (MDS).
• Educate, train and assist nursing staff regarding assessments, planning, implementation and MDS processes.
• Ensure all current MDS/PPS updates are implemented and staff is in-serviced.
• Cross-train between PPS and OBRA.
• Conduct regular resident interviews as required by the MDS.
• Respond to clinical emergencies, regulatory visits, surveys and issues outside of normal hours.

MDS Coordinator

The primary role of the MDS Coordinator is to coordinate and guide the overall process and tracking of all Medicare/Medicaid case-mix documents, and care assessments in order to assure appropriate reimbursement for services. The position is also responsible for completing concurrent MDS to assure achievement of maximum allowable RUG categories, as well as integrating information from Nursing, Dietary, Social Services, and Rehabilitation departments for appropriate reimbursement.

Responsibilities:

• Coordinates the assessment, planning, implementation and evaluation of resident care by following the MDS process.

• Coordinates and facilitates care conferences with the interdisciplinary team.

• Coordinates, completes and reviews Minimum Data Sets (MDS)

• Educates, trains and assists nursing staff and other departments regarding the roles of assessments, planning, implementation and MDS process; and completion of applicable sections of MDS.

• Responsible for ensuring all current MDS/PPS updates are implemented and staff is in-serviced.

• Will cross train between PPS and OBRA.

• Conducts regular resident interviews as required by the MDS

• Available to respond to clinical emergencies, regulatory agency visits, surveys and issues outside of normal work hours as needed
