Greenwich Woods Rehabilitation seeks a full-time MDS Coordinator who is a licensed Registered Nurse in Connecticut with experience completing resident assessments in a long-term skilled nursing setting. The role requires overseeing assessments, scheduling, and transmitting to the State DOH to ensure regulatory compliance.

The position involves coordinating care plans, initiating Medicare coverage for new residents, and staying current with coverage requirements.

Qualifications

• Licensed CT RN with MDS experience in long-term care.
• Detail oriented with excellent communication and organizational skills.
• Ability to work cooperatively with our facility interdisciplinary team.
• Knowledge of state and federal regulations related to long term care nursing reimbursement.
• Ability to work flexible hours when necessary.

MDS Coordinator / Registered Nurse

Greenwich Woods Rehabilitation seeks a full time MDS Coordinator. Our beautiful facility has served the Fairfield and Westchester Counties for more than thirty years and is located within proximity to all major highways.

Candidate must be a licensed Registered Nurse in the State of Connecticut with previous experience in the completion of resident assessments in a long-term skilled nursing setting.

RESPONSIBILITIES

• Oversee and facilitate the completion of resident assessments in accordance with Federal and State regulations
• Establish assessment schedules and ensure that assessments are done in an accurate and timely manner
• Transmitting assessments to the State of Connecticut, Department of Health
• Coordinate resident care plans in accordance to regulatory requirements, ensuring that resident care is delivered effectively
• Initiate Medicare coverage for newly qualified residents and sending denial letters when necessary
• Remain updated on changes in Medicare coverage and help determine documents necessary for reimbursement.

QUALIFICATIONS

• Experienced Registered Nurse with knowledge of the MDS process
• Must possess a Registered Nurse license in the State of Connecticut
• Detail oriented with excellent communication and organizational skills
• Ability to work cooperatively with our facility interdisciplinary team
• Flexible and decisive, with the ability to work under pressure
• Working knowledge of state and federal regulations as they relate to long term care nursing reimbursement
• Ability to work flexible hours, when necessary
• Ability to communicate effectively with both a diverse clientele and workforce through sound business decisions, diplomacy and discretion
