Job summary

Nexion Health Management in Flower Mound, TX seeks an MDS Coordinator. You will manage MDSs, care plans, and documentation to ensure high-quality resident care and regulatory compliance. This role requires accuracy, teamwork, and strong clinical knowledge.

Ideal candidates hold RN or LVN/LPN licensure, have SNF experience with MDS/Care Plans, and demonstrate proficiency in RAI/PPS processes. Excellent communication and organizational skills are essential to succeed in this position.

Qualifications

• Current RN or LVN/LPN license in state of practice.
• Graduate of an accredited school of nursing.
• 1+ year experience in skilled nursing facility with MDS/Care Plans as MDS Coordinator.
• Proficiency in the RAI / PPS processes.
• Strong attention to detail, documentation and resident assessment skills.
• Good communication, organizational and computer skills.
