Job summary

Rosewood Center for Rehabilitation & Healing in Lake Worth Beach, FL is seeking an experienced MDS Coordinator to lead accurate MDS assessments and care plans, ensure regulatory compliance, and support resident-centered care within a strong interdisciplinary team.

The role requires RN or LPN licensure, 1–2+ years of MDS experience in a skilled nursing facility, and proficiency with MDS 3.0/RAI and EHRs, along with excellent communication and organizational skills.

Qualifications

• Active RN or LPN license required.
• 1–2+ years of MDS experience in a skilled nursing facility.
• Strong knowledge of MDS 3.0, RAI, and regulatory requirements.
• Proficiency with EHR systems and clinical documentation.

Responsibilities

Rosewood Center for Rehabilitation & Healing is seeking an experienced and detail-driven MDS Coordinator to join our clinical team. This is an exciting opportunity to play a key role in resident care, drive quality outcomes, and collaborate with a strong interdisciplinary team.

Compensation: $85,000 - $110,000 annually DOE

Key Responsibilities

• Lead and manage timely, accurate MDS assessments and care plans

• Ensure compliance with state and federal regulations and guidelines

• Collaborate closely with clinical teams to support individualized, resident-centered care

• Review and audit documentation for accuracy and completeness

• Provide guidance and training on MDS processes as needed
