Job summary

Healthcare of Tennessee in Dickson, TN is seeking an MDS Coordinator to ensure accurate data collection and timely submissions for resident assessments. You will collaborate with care teams to support high-quality, individualized care plans and maintain meticulous documentation.

In this role, you will analyze MDS data for trends, participate in quality improvement initiatives, and help ensure compliance with regulatory standards while communicating clearly with residents, families, and the care

Responsibilities

• Coordinate the collection, validation, and submission of MDS assessments, care plans, and resident information in a timely and accurate manner.
• Collaborate with healthcare providers, therapists, and other team members to ensure comprehensive care plans meeting each resident's unique needs.
• Review and analyze MDS data to identify trends, patterns, and areas for improvement, and implement changes as needed.
• Develop and maintain effective relationships with residents, families, and healthcare providers to ensure excellent customer service and communication.
• Participate in quality improvement initiatives and regulatory compliance efforts to maintain our high standard of care.
• Maintain accurate and detailed records of all MDS assessments, care plans, and resident information.
