Job summary

Cooney Healthcare in Helena, MT is seeking an experienced MDS Coordinator to join our skilled nursing facility. You will oversee resident assessments to ensure high-quality care and accurate documentation, while upholding Federal and State regulations.

The role involves coordinating with nursing, PT, and nutrition teams to develop individualized care plans and maintain compliance. This in-person position lists RN or LVN license as required and prefers at least 1 year of MDS coordination

The MDS Coordinator will oversee and facilitate the completion of resident assessments at this nursing facility. Throughout the assessment and while planning care, they ensure compliance with Federal and State regulations.

MDS Coordinator Responsibilities:

Assess residents behavioral, psychological, clinical, nutritional, and physical strengths and needs to create an individualized cared plan that considers the resident holistically. The MDS Coordinator also works with nursing, PT, nutrition, etc., to ensure care plan guidelines for each resident are followed, and compliant with ethical standards and Medicare requirements.

MDS Coordinator Requirements:

• RN or LPN license required


• Experience working in a Skilled Nursing Facility.


• Must be able to pass a background check.
