Job summary

Northern Pines Rehabilitation and Nursing in Cut Bank, MT, is seeking an experienced MDS Coordinator to oversee resident assessments, coordinate comprehensive care plans, and help ensure compliance with Federal and State regulations. This full-time role emphasizes quality resident care and adherence to Eduro HealthCare values.

The position requires RN licensure (preferred) and prior Skilled Nursing Facility experience.

Qualifications

• Experience in a Skilled Nursing Facility is desirable.
• Must be able to pass a background check.

Responsibilities

• Assess residents' behavioral, psychological, clinical, nutritional, and physical needs to create individualized care plans.
• Collaborate with nursing, PT, nutrition, and other teams to ensure care plan guidelines are followed and compliant with Medicare requirements.
• Oversee and facilitate completion of resident assessments and ensure compliance with Federal and State regulations.

The MDS Coordinator will oversee and facilitate the completion of resident assessments at nursing facilities. Throughout the assessment and while planning care, they ensure compliance with Federal and State regulations.

MDS Coordinator Responsibilities:

Assess residents behavioral, psychological, clinical, nutritional, and physical strengths and needs to create an individualized cared plan that considers the resident holistically. The MDS Coordinator also works with nursing, PT, nutrition, etc., to ensure care plan guidelines for each resident are followed, and compliant with ethical standards and Medicare requirements.

MDS Coordinator Requirements:

• RN license preferred
• Experience working in a Skilled Nursing Facility.
• Must be able to pass a background check.
