Job summary

Ontario Center for Rehabilitation and Healthcare in Canandaigua, NY seeks a qualified MDS Coordinator to complete MDS 3.0 assessments and care plans, initiate care planning activities, and distribute monthly calendars. This on-site role requires careful documentation and collaboration with the nursing team.

Ideal candidates are highly organized, possess a NY RN license, have long-term care experience, strong computer and communication skills, and work well in a team to maintain accurate records

Qualifications

• Prior MDS 3.0 experience required.

Responsibilities

• Complete MDS 3.0 assessments and care plans as assigned.
• Initiate care plans and supporting activities as assigned.
• Create and distribute monthly care plan calendars in a timely fashion.
• Maintain and update all care plans and assessments as required.
• Monitor and audit clinical records for accuracy and timeliness.
• Inform DON of persistent issues related to non-compliant documentation.
• Protect confidentiality of resident and facility information at all times.

DUTIES:

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records, ensuring accuracy & timeliness
• Informing DON of persistent issues related to non-compliant documentation
• Protecting the confidentiality of Resident & Facility information at all times

REQUIREMENTS:

• MUST HAVE PRIOR MDS 3.0 EXPERIENCE
• Valid New York RN License
• Long Term Care Experience Required!
• Must be highly organized, professional & motivated
• Should have solid computer skills
• Excellent communication skills
• Should be friendly and a team worker
