$29.00/hr - $45.00/hr

Job Summary

Ormsby Post Acute Rehab is seeking a MDS Coordinator Nurse to coordinate the development and completion of the Resident Assessment Instrument (RAI), including the Minimum Data Set (MDS), Care Area Assessments (CAAs), and Care Plan, in accordance with State and Federal requirements. Participates as part of the Nursing Center Interdisciplinary Team (IDT) for Medicare management as assigned.

Essential Functions
  • Coordinate the IDT in timely completion of assessments.
  • Utilize MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs).
  • Communicate ARD selection for specific MDSs to IDT.
  • Complete Care Area Assessments (CAAs) as required.
  • Complete care plan items relevant to current MDS and attend care plan meetings as assigned.
  • Monitor documentation in the medical record to support MDS coding.
  • Participate in Medicare utilization management as assigned.
Education, Licensure, and Experience
  • Required: Current RN Licensure in the State of Employment.
  • Required: Computer aptitude with EMR systems.
  • Preferred: Two years’ experience in long-term care.
  • Preferred: Experience with RAI process and MDS certification.
  • Preferred: Point Click Care (PCC) experience.
