Job summary

Brooke Knoll Village in Avon, IN is seeking a licensed LPN or RN to join our team as an MDS Coordinator. The role requires on-the-job training or completion of an MDS-training program, with opportunities to train quick learners.

The MDS Coordinator will manage OBRA/PPS/Manged Care schedules using RAI guidelines, coordinate care plans with the interdisciplinary team, and ensure accurate ICD-10 coding and CMS submissions while supporting audits and staff education.

Qualifications

• Licensed LPN or RN in Indiana.
• On-the-job training or completion of an MDS-training program.
• Organized, detail-oriented with strong assessment and communication skills; capable of independent work and teamwork.
• Ambitious, inquisitive and eager to learn.

Responsibilities

• MDS scheduling and completion for OBRA/PPS/Managed Care using RAI guidelines.
• Assist with care plan scheduling and completion with the interdisciplinary team.
• Assign and update ICD-10 codes based on physician diagnoses on admission and with each MDS.
• Transmit OBRA/PPS MDS Assessments to CMS per Federal Guidelines.
• Complete certifications and recertifications for Medicare Part A beneficiaries.
• Oversee and monitor MDS documentation and charting requirements.
• Educate staff on MDS processes as needed.
• Assist with data collection for audits (State, Medicare, Managed Care).
• Participate in facility and company meetings; apply RAI rules and monitor QMs.

About the Role:

Brooke Knoll Village is seeking an MDS Coordinator to-

• MDS scheduling and completion for OBRA/PPS/Managed Care utilizing RAI guidelines
• Assist with care plan scheduling and completion with the interdisciplinary team
• Assign and update ICD-10 codes based off physician diagnosis on admission and with each MDS
• Transmit OBRA/PPS MDS Assessments to CMS per Federal Guidelines
• Completion and Certifications/Re-certifications when a resident is receiving Medicare Part A Benefit
• Oversee and monitor MDS documentation and charting requirements needed to support services provided
• Educate staff on MDS processes as needed
• Assist with data collection for audits including but not limited to, State, Medicare, and Managed Care.
• Participate in facility and company meetings per policy
• Become proficient with and apply RAI rules/regulations
• Become familiar with QMs and assist team in monitoring
