Job summary

Responsibilities

• Coordinate and conduct MDS assessments, care plans, and quality measures for residents in our skilled nursing facility.
• Ensure timely and accurate completion of MDS assessments and care plans, meeting deadlines and quality standards.
• Collaborate with interdisciplinary teams, including physicians, nurses, and therapists, to develop and implement individualized care plans.
• Maintain accurate and detailed records of resident assessments, care plans, and quality measures.
• Analyze data and identify trends to improve care processes and outcomes.
• Provide education and training to staff on MDS and care planning processes.
• Participate in quality improvement initiatives and quality management activities.
• Ensure compliance with federal, state, and accrediting agency regulations and standards.
