Job summary

Qualifications

• RN or LPN license is required.
• Long-term care experience is required.
• Restorative Certification required within six months of hire if not already certified.
• Ability to read, write, speak and understand English.

Responsibilities

• Performs MDS assessments and ensures timely, accurate submissions.
• Coordinates care planning and resident assessment with interdisciplinary teams.
• Schedules and participates in care conferences and IDT meetings.
• Monitors quality measures and documentation accuracy.
