Job summary

Qualifications

• Graduate of an accredited school of nursing with RN or LVN licensure.
• Minimum of two years healthcare experience.
• Experience with MDS/RAI processes is highly desirable.
• Able to coordinate interdisciplinary teams and communicate effectively.

Responsibilities

• Coordinate the Resident Assessment Instrument (RAI) process for accurate and timely resident assessments.
• Ensure documentation supports services and payer requirements.
• Communicate with interdisciplinary team to optimize care plans.
• Maintain compliance with CMS/Medicare/Medicaid guidelines and RIHS policies.
• Manage MDS schedules, ARD dates, and data integrity across the facility.
