Journey is seeking a dedicated MDS Assistant in St. Matthews, KY to support the Resident Assessment Process and care planning for residents.

This on-site, full-time role requires a licensed RN and strong knowledge of MDS 3.0, ensuring the highest quality outcomes for residents and regulatory compliance. You will collaborate with the Care Team to develop and monitor individualized Care Plans, contributing to exceptional care standards and positive resident experiences in a community-focused

Qualifications

The MDS Assistant participates in the Resident Assessment Process through completion of the pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs). Participates in developing individualized resident Care Plans, identifying the needs and projected outcomes as required by Federal and State regulations.

Required Qualifications

• A professionally trained Registered Nurse that has earned a certificate/diploma or degree in nursing.
• Must hold a current nursing license in the state of employment.
• Working knowledge of the MDS 3.0 (current version of minimum data set).
